A medical condition is a chronic illness that may oftentimes prevent an individual from performing normal daily tasks during thier lifetime. An individual with a medical condition may require as needed or daily treatment for thier medical condition. If your student has a medical condition that requires further treatment while at school it will be important to work with your Family Physician and the School Nurse to develop a treatment plan that works best for your student. You can review and print the forms below to get started on a treatment plan for your student. Medications needed for the treatment plan will require a Physician signature. When the forms are completed by a Physician and Parent/Guardian, please return to the School Nurse with medications.
